Change of Address 

If you change addresses, please inform the Office of Student Affairs within five (5) days of your occupancy so that important information, such as your child's school assignment, or any new information, can reach you. 

 
To update your address, return to the Office of Student Affairs with:
  1. Affidavit of Residency (click to download
  2. Valid Photo ID (as listed below)
  3. New deed, settlement statement, mortgage statement or lease as listed below (and if a lease a new notarized Landlord Living Agreement (click to download)
  4. One (1) utility bill for your new Brookline address listed below

  • You may also email a scanned copy of these documents to residency@psbma.org.
  • If your new address is in a different school zone, you may need to attend your new neighborhood school.
  • If you are a parent with whom the children do not primarily reside, but you have moved, please inform the Office of Student Affairs so that we can update your address.
