Residency Verification

Residency Verification processes are conducted annually per the Brookline School Committee Policy.  

Following initial enrollment, a student’s parent or legal guardian must provide a signed evidence of continued residency annually as outlined below:
  1. Affidavit of Residency (click to download) 
  2. Valid Photo ID
  3. One (1) mortgage statement or property tax bill or lease as listed below (No Landlord Living Agreement will be required for Residency Verification)
  4. One (1) utility bill for your Brookline address as listed below

Proofs can be scanned and emailed to residency@psbma.org.  They can also be hand-delivered to: the Office of Student Affairs at 24 Webster Place., or mailed to: Office of Student Affairs, 333 Washington St., Brookline, MA 02446.
